Sweating is an important bodily function. The sweat evaporation has a cooling effect on the skin surface, therefore providing protection in a hot environment. Exercise related sweating provides a “coolant” effect as well, protecting the body from overheating during a heavy workout.

Excessive sweating is caused by “out of control” overactive sweat glands regardless of ambient temperature or exercise. Stressful situations augment the problem.

Usually isolated body areas are involved, most frequently the underarms, palms, feet, and face. This condition varies from a minor, though persistent, discomfort to disabling social and/or handicap. Therapy for Hyperhidrosis can be challenging for both the patient and the physician. The treatment can be medical and surgical.

Medical Treatment

Topical Agents
Boril Acid, 2.5% tannic acid solutions, Resorcinol, Glutaraldehyde. All of these agents are limited by staining, contact sensitization, irritancy or limited effectiveness.

The first line topical agent is Drysol. It should be applied nightly on dry skin with or without occlusion until a positive result is obtained.

Underarm Hyperhidrosis may be treated with aluminum Chloride Gel, although the Gel may cause mild cutaneous irritation.

Botulinum Toxin – Botox Injections are effective, because Botox blocks the connection between the overactive nerves and the sweat glands. The effect lasts for 4-12 months. Most frequently used in underarm, palmar and groin Hyperhidrosis.

Surgical Treatment

Surgical Treatments are more invasive procedures with long lasting results

Thoracic Sympathectomy is a video assisted minimally invasive surgery under general anesthesia, in hospital. The sympathetic nerve chain is interrupted along the upper part of the thoracic spine. This area is in control of the hand and underarm Hyperhidrosis. By interrupting this nerve chain, the overactive nerve input into the sweat glands are turned off. The effect is immediate, and long lasting, with 90% complete cure.

Complications are rare and not severe. Occasionally compensatory sweating- sweating in previously unaffected areas of the body. Mild/moderate chest wall pain, usually temporary.

Laser Liposuction of the armpit

Long term efficacy. The procedure is done under local anesthesia in medical offices, clinics or surgery centers.
